Cefcapene is a third-generation cephalosporin antibiotic.

Cefcapene Pivoxil Hydrochloride is an antibacterial agent that has demonstrated activity against both gram-positive and gram-negative bacteria. The compound has documented antibacterial activity at 90% in vitro inhibition of Staphylococcus aureus A, B, C, F, and G streptococci and Streptococcus pneumoniae isolates at a concentration of less than or equal to 2μg/ml and 0.12μg/ml respectively. In the same study 100% of the isolates of Haemophilus influenzae were inhibited at concentrations equal to or less than 0.06μg/ml.

Cefcapene pivoxil hydrochloride is an oral 3rd-generation cephalosporin with a wide spectrum of activity. It has been in use since 1997 only in Japan. As well as gastrointestinal symptoms, it has been reported to the manufacturer as causing rashes. There are 5 b-lactams  that have a pivaloyloxymethyl base as a side chain, e.g., pivmecillinam, cefteram pivoxil, cefetamet pivoxil, cefditoren pivoxil and cefcapene pivoxil hydrochloride .
